berryFORMS turns your Blackberry® Smartphone into efficient data collection device, for taking surveys, filling in forms and performing systematic counts. It even supports the capturing of pictures and GPS coordinates in your forms. After creation, templates can be efficiently distributed among as many  users as desired through our web based portal. It is simple to use, simple to deploy, and easy to get started immediately. The data you collect can be distributed/exported immediately in CSV (semicolon delimited), .XML or Excel format using Blackberry® E-mail. No BES is required.

Just follow these simple steps :

1 – Register your account

• There's no charge to activate an account, so get started now.
• Complete the registration form. Click here to go to the registration form
• You will receive an email soon confirming your registration and with detailed instructions guiding you through the next steps outlined below

2 – Build your Free Trial Survey

• Login into your account and fill out the New Survey Form
• Create a new survey or use a survey template
• If you prefer, you can also fax your trial survey/form to 954-252-4127 or e-mail to survey@berryforms.com

3 – Publish and Collect

• Once your survey is built, berryFORMS will notify you that your survey/form is ready to be used with your Blackberry  and will send you an email to help install the Survey Client on your device
• Then just start collecting information on your device and just like that you are paperless

4 – Collect Responses and Analyze Results

•  Receive an e-mail with one click where you can download a summary of your results in multiple formats (MS Excel, CSV or XML format )
• View your results as they are collected in real-time.Securely share your survey results with others. Powerful filtering with live graphs and charts.

5 – System Requirements

•  BlackBerry smartphone (BIS and BES)
•  BlackBerry OS 4.0 or higher
•  BlackBerry OS 4.2 or higher required for Photo Template functionality
•  BlackBerry smartphone model with camera or SD card required for Photo Template functionality
